Project Samaritan AIDS Services C.O.B.R.A. Case Management

Our C.O.B.R.A. Case Management program helps people with HIV/AIDS and their families get the services they need to remain healthy and independent. The C.O.B.R.A. team offers comprehensive Medicaid case management and referrals to/advocacy with concrete supportive services including:

  • Primary medical care
  • Substance abuse treatment
  • Counseling
  • Nutritional services and food pantries
  • Housing services
  • HIV/AIDS Services Administration (HASA)
  • Legal services
  • Permanency planning

Requirements for Admission to the C.O.B.R.A. program include

  • HIV+/AIDS diagnosis
  • Medicaid eligibility
  • Need for intensive services
