Samaritan AIDS Services C.O.B.R.A. Case Management
Our C.O.B.R.A.
Case Management program helps people with HIV/AIDS and their families
get the services they need to remain healthy and independent. The
C.O.B.R.A. team offers comprehensive Medicaid case management and
referrals to/advocacy with concrete supportive services including:
- Primary
medical care
- Substance
abuse treatment
- Counseling
- Nutritional
services and food pantries
- Housing
services
- HIV/AIDS
Services Administration (HASA)
- Legal services
- Permanency
planning
Requirements
for Admission to the C.O.B.R.A. program include
- HIV+/AIDS
diagnosis
- Medicaid
eligibility
- Need for
intensive services
