CABRINI HOSPICE
A Program of Cabrini Medical Center

The philosophy of Cabrini Hospice is built on the foundation of heartfelt commitment to caring for individuals with advanced cancer, AIDS, and other illnesses. The goal is to help patients in their homes for as long as possible. Services are available in Manhattan, the Bronx, Brooklyn and Queens. A program is developed for each patient and care-partner by the hospice team and the necessary medical and support services are provided. A 15-bed inpatient unit specially designed to foster a comfortable homelike atmosphere is available for patients when short-term, acute hospital care is necessary. Cabrini Hospice is one of the few area programs offering pediatric hospice care.

Referrals
Referrals are accepted from anyone who identifies the need for hospice care.

COMPREHENSIVE COMMUNITY
HOSPICE

 

Comprehensive Community Hospice (CCH) recognizes the importance of self-determination and respect for the individual in all aspects of care.   CCH will provide a continuum of palliative and supportive care for the patient, as well as support, education, and assistance to their loved ones.  An experienced team of professionals and volunteers will work to address the unique physical, emotional, psychological , spiritual, and bereavement needs of each patient and family, from admission, until after discharge.  Care is provided in the home, and inpatient management of difficult to control symptoms is available.

CCH provides aggressive pain management and symptom control, and encourages the patient and their loved ones to be active in determining treatment options.

Comprehensive Community Hospice serves Queens and Nassau Counties.
